If a therapist’s livelihood is based on a patient’s weekly appointments, does the therapist have any incentive to be honest and objectively help the client? Or, do they just say what the patient wants to hear.

The therapist I work with always shakes my hand whenever we greet each other, or when I leave. Today is the first time in 8 months he didn't shake my hand when I left. Could this be because I told him I had a cold last weekend? I feel awful about it.
